Documents Needed for Enrollment

• Birth certificate - Must be original
• Transcripts from previous schools
• Current vaccination shots
• Original Social Security Card
• Notarized Guardianship paperwork
• IEP paper work
• Proof of Residence with Street Address and P.O. Box     (Water, Light, or Gas Bill)

Letter to Parents/Guardians

Hello, Berino's parents,

These are the links needed to complete the request for access to the parent portal. You will need to complete the Parent Portal Application and Agreement forms. Links are included in both English and Spanish for your convenience. You will need to complete both forms and return them to the school with a copy of your state issued identification. To do this quickly and safely, you have many options on how to return your documents:

You can email a photo of the forms with your ID to Monique Gonzales (Data Secretary) at mggonzales@gisd.k12.nm.us.

There is also the option of faxing the required information to (575) 882-7249.
You can also deliver the forms in person on Monday, Tuesday, Thursday and Friday from 7:30 to 3:30 at the school.
You will need to turn in these forms in order to access your child's school information. Such as student grades, report cards and attendance.

Hola, padres de Berino,

Estos son los enlaces necesarios para completar la solicitud de acceso al portal para padres. Deberá completar los formularios de Solicitud y el  Acuerdo del Portal para Padres. Para su conveniencia, los enlaces están incluidos en inglés y español. Por favor no se le olvide completar ambos formularios y devolverlos a la escuela con una copia de su identificación emitida por el estado. Para hacer esto de forma rápida y segura, tiene varias opciones sobre cómo devolver estos documentos:

Puede enviar por correo electrónico una foto de los formularios con su identificación a Monique Gonzales (Secretaria de datos) a mggonzales@gisd.k12.nm.us.

También existe la opción de enviar por fax la información requerida al (575) 882-7249.
Además, puede entregar los formularios personalmente los lunes, martes, jueves y viernes de 7:30 a 3:30 en la escuela.
Deberá entregar estos formularios para poder acceder a la información de la escuela de su hijo. Tales como calificaciones de los estudiantes, boletas de calificaciones y asistencia.
